Respite Care
Our respite care services offer temporary relief for family caregivers, allowing them to take a break and recharge.
Transitional Care
Our transitional care program helps patients transition from hospital to home, with a focus on medication management and follow-up care.
Social Activities & Outings
This service includes support and/or supervision for individuals to participate in meaningful activities in the community. Activities may include, but are not limited to:
- Attending community events or religious services
- Visiting parks, libraries, or museums
- Going to the movies or restaurants
- Participating in hobby groups or clubs
- Shopping for personal or household needs
- Visiting with friends or family in the community
